* Original Form W-8BEN-E “Certificate of Status of Beneficial Owner for United States Tax Withholding and Reporting (Entities).”
* Copy of the Fiscal Identification Document.
* Bank reference (issued within the past 90 days) or last three (3) bank statements.
* One (1) commercial reference (with date, name, relationship and phone number).
* Copy of a service receipt (e.g., telephone, water, electricity, gas, cable TV) dated from the preceding 90 days. If the service receipt is not in the name of the company, you must submit an explanatory letter (with date of issue, company name, address indicated on the service receipt, name of service holder, relationship between applicant and service holder).
– Note: The address reflected in the service receipt must match the one indicated in the Application for New Account for Companies and the form W-8BEN-E.
* Brief description of the business (name of business, address, opening date, number of employees, number of branches, description of commercial activity, main suppliers, regular customers, and payments to be made through the account).
* Constitutive act or trade register.
* Shareholder meeting minutes and/or Board of Directors meeting minutes.
* Board of Directors meeting minutes authorizing account signatories
* Company financial statements for the last fiscal year (balance sheet and profit and loss statement). If the company generates revenues of more than $1,000,000, the financial statements must be audited. If the company generates revenues less $1,000,000, the financial statements must be certified by a Certified Public Accountant (CPA).
Copy of the company’s last income tax return.
AUTHORIZED SIGNER
Each authorized signer must provide the following current documentation: one (1) primary identification or two (2) forms of secondary identification, outlined below:
* Primary identification with government-issued photo: Copy of passport.
* Secondary photo Identification:
– Copy of the national identity document.
– Copy of labor identification.
– Copy of driver’s license.
* In the case of NEW COMPANIES, i.e., companies created less than or equal to one (1) year ago, which at the time of account opening cannot produce financial documentation as established in the above mentioned requirements, the company’s legal owner(s) and/or legal representative(s) must provide the following documentation: (1) bank reference, personal balance, and income tax return (if any).
